About the programme

The IMpulse CVD Programme aims to support primary care teams to develop confidence in the identification and management of people with risk factors such as atrial fibrillation, hypertension and hypercholesterolaemia which increase the likelihood of developing cardiovascular disease. The programme will also support teams in optimising the management of people with established cardiovascular disease e.g. with heart failure or following a heart attack or stroke.

Cardiovascular disease (CVD) prevention is a priority area within the NHS Long Term Plan, which sets out a clear ambition to prevent 150,000 strokes, heart attacks and dementia cases over the next ten years. Targeted CVD prevention initiatives directly reduce health inequalities and are highly cost effective in preventing cardiovascular events.

The Health Innovation West Midlands (HIWM) Cardiovascular Disease (CVD) Prevention and Management Programme has been established to support organisations, acute Trusts and integrated care systems (ICSs) and integrated care boards (ICBs) to implement new or modified models of care to reduce cardiovascular morbidity and mortality, address and reduce health inequalities and improve the overall health and well-being of the population in the West Midlands.

HIWM collaborate with ICB Education Hubs and ICBs in this local project in providing practical upskilling in the detection and management of atrial fibrillation, hypertension, lipid manager and hypercholesterolaemia, and heart failure workshops for all healthcare professionals in primary care and other sectors who are involved in the diagnosis and management of CVD which includes GPs, Practice Managers, Nurses (community, practice-based, hospital), health care assistants, pharmacists (community, practice-based, hospital), social prescribers, health and wellbeing coaches, paramedics, physicians associates etc.

This project will link in with aims of the HIWM’s lipid optimisation programme, the blood pressure optimisation programme, the InHIP programme, the heart failure programme, the preventative cardiology programme and the cardiac rehabilitation programme.

Project ambitions

To enable primary care colleagues to feel confident in the diagnosis and management of cardiovascular conditions through:

  • Increasing their knowledge and understanding of each condition
  • Increasing their confidence in how to manage patients
