HIWM support drives success in heart failure innovation
Health Innovation West Midlands (HIWM) is spotlighting its role in a pioneering heart failure project that’s earned national and international recognition.
As part of its commitment to improving patient outcomes through innovation, HIWM supported four regional sites in securing NHS England funding for the Managing Heart Failure at Home (MHF@Home) pilot programme. Among these was University Hospitals Coventry and Warwickshire (UHCW), which implemented a unique palliative care model for patients with advanced heart failure and a prognosis of one to two years.
The project, led by UHCW Advanced Clinical Practitioner, Stephanie Kirkland, aimed to reduce hospitalisations and readmissions through remote monitoring and personalised care. HIWM played a key role in shaping the project’s success, providing strategic guidance, mentorship, and support in defining metrics to demonstrate impact.
This collaborative effort has led to remarkable achievements. Stephanie was recently awarded the Nursing and Allied Professional Investigator Award at the World Congress on Heart Failure in Belgrade, where she presented the outcomes of the UHCW pilot. She also received the Runner-Up Award for Best Abstract Oral Presentation at the British Cardiovascular Society Conference in Manchester.
Stephanie says: “I am thrilled with the recognition that the project has reached international and national audiences. I am passionate about improving palliative and end of life care for heart failure patients. Health Innovation West Midlands have participated and supported me in this ambition, through mentorship, networking and benefits realisation. At the start of the project, I was a novice in leadership, however my confidence has grown with their professional support.”
The success of the MHF@Home project underscores HIWM’s role in empowering healthcare professionals to lead innovation and improve care pathways. Since the awards, Stephanie has been invited to advise heart failure teams in Belgium and contribute to the development of a national heart failure palliative care framework.
This achievement highlights how regional innovation networks like HIWM can drive meaningful change, locally, nationally, and globally, by supporting the health system workforce to adopt and scale impactful innovations.
