IMpulse Cardiovascular Disease (CVD)
About the programme
The IMpulse Cardiovascular disease (CVD) Programme aims to support primary care teams to develop confidence in the identification and management of people with risk factors such as atrial fibrillation, hypertension and hypercholesterolaemia which increase the likelihood of developing CVD. The programme will also support teams in optimising the management of people with established CVD e.g. with heart failure or following a heart attack or stroke.
CVD prevention is a priority area within the NHS Long Term Plan, which sets out a clear ambition to prevent 150,000 strokes, heart attacks and dementia cases over the next ten years. Targeted CVD prevention initiatives directly reduce health inequalities and are highly cost effective in preventing cardiovascular events.
The Health Innovation West Midlands (HIWM) CVD Prevention and Management Programme has been established to support organisations, integrated care boards (ICBs), acute Trusts and integrated care systems (ICSs) to implement new or modified models of care to reduce cardiovascular morbidity and mortality, address and reduce health inequalities and improve the overall health and well-being of the population in the West Midlands.
HIWM collaborates with ICB Education Hubs and ICBs in this local project to provide practical upskilling in the detection and management of atrial fibrillation, lipid management and hypercholesterolaemia, hypertension and heart failure workshops for all healthcare professionals in primary care and other sectors who are involved in the diagnosis and management of CVD. This includes GPs, Practice Managers, Nurses (community, practice-based, hospital), health care assistants, pharmacists (community, practice-based, hospital), social prescribers, paramedics, physicians associates, health and wellbeing coaches etc.
This project will link in with aims of the HIWM’s lipid optimisation programme, the blood pressure optimisation programme, the InHIP programme, the heart failure programme, the preventative cardiology programme and the cardiac rehabilitation programme.

To enable primary care colleagues to feel confident in the diagnosis and management of cardiovascular conditions through:
- Increasing their knowledge and understanding of each condition
- Increasing their confidence in how to manage patients
